Join us at the 14th annual Heartland Conference on Health Equity and Patient Centered Care! The Heartland Conference is the largest health equity conference in the region, bringing together over 300 participants from across the Midwest. The conference aims to highlight partnerships and encourage open discussion among academic health centers, community health workers, faith-based community, and community-based service organizations.

Regional Determinants of Health: Radically Uniting for Healthy Communities.

Objectives

  • Develop health-focused partnerships that address issues of health equity
  • Explore strategies to preserve the health of our communities
  • Understand perspectives, and address community and population health
  • Provide opportunities for community health workers to grow and develop
  • Highlight patients and families as health care partners
